Your first visit to Transformation Wellness Center establishes a vital foundation for our relationship with you. We take the time to obtain important background information, including your medical history, and allow you to get to know your practitioner .

Please print and fill out the Health Intake Form if you are scheduled for massage therapy so we can expedite your first visit:. You can also print the information on the BioMat below, as you may be interested in the additional health benefits that it provides. The Far Infrared Bio-Mat is an add-on option that allows you to have basically two sessions in one for only $10 or $15 with a 90 minute session.

Conveniently located near M-59 and I-75 freeways for easy access.

Our suite is located on the north side of E. Long Lake Road just east of Rochester Road in the Long Lake Plaza with the Secretary of State office. Our suite is located in the northeast corner of the plaza between the Steelhouse Tavern and Boy Scouts of America Scout Shop.  Our entry door is located under the “1147” address sign.
